This diagram shows how the mechanics of the heart co-ordinate with the heart sounds and the ECG (EKG).

By keeping this diagram in mind, it can make identifying murmurs much easier, by taking the pulse whilst auscultating.  

The pulse represents the maximum arterial pressure, which as you can see in the diagram occurs in between heart sounds 1 and 2. Once you have identified which heart sound is which, you can more easily identify systole, and diastole, and describe with greater accuracy the nature of a murmur if present.

Here’s some causes of murmur patterns:

Ejection Systolic Murmurs

AorticStenosis - Ejection systolic murmur, radiates to carotids (In Aortic Sclerosis, there is no carotid radiation)

Pulmonary Stenosis  - Ejection systolic murmur which radiates to back/left scapula (may obscure second heart sound)

Atrial Septal Defect - sounds like pulmonary stenosis

Hypertrophic Cardiomyopathy - Harsh Ejection systolic murmur, palpable

Pansystolic Murmurs

Mitral Regurgitation - Best heard at apex, radiates to axilla

Tricuspid Regurgitation - Best heard at left lower sternal edge (tricuspid region) 

Ventricular Septal Defect - Best heard at left sternal edge

Early Diastolic Murmurs

Aortic Regurgitation - best heart at left sternal edge, in expiration, with patient leaning forward

(Rarely) Pulmonary Regurgitation

Mid-Diastolic Murmurs

Usually Mitral Stenosis - Best heard with Bell of stethescope at apex, with patient rolled to left

Rarely Tricuspid stenosis - sounds simliar to Mitral Stenosis

Continuous Murmur - referred to as a ‘machinery murmur’, although I don’t think anyone knows what machine it sounds like.

Patent Ductus Arteriosus - rare in adults. Best heard over upper left sternal border, radiates to back/left scapula

> Nurse pouch: praise whoever decided to modify the bum bag for nurses because this thing holds all of your stuff AND you can organise it too (well at least before a chaotic shift when you’re just cramming things in at frantic speeds!
> Haemostats: these things come in handy ALL THE TIME especially for stuck fluid lines to take doggos for walks
> Stethescope: I know people froth over the littmans and it’s a perf Christmas present idea for your parents, unfortunately mine got stolen and I had to swap it with a dodge cheap one but so far it’s just as good as my littman :) so if you can’t afford the 2-300 littman a 80-120 Stethescope can get you through :3
> Vial opener thingy: after I cut myself opening a vial of Valium one time I brought this straight away, you put the top into the end of it and it opens the vial with no risks of cuts or glass shards in your fingers :D
> Calculator: I am guilty of whipping out my phone for calcs but it does look unprofessional so carrying a calculator is a good way to get out of the habit
> Penlight/thermometer: because I kept accidentally stealing clinics pen lights and thermometers and so far this has been the best way to prevent me nicking stuff!
> Cheat book: this is a GODSEND for smallies, even for studying. It fits in your pocket and has excellent flowcharts and summaries for absolutely anything dog/cat related
> Hand cream: I honestly didn’t realise how much your skin can dry out from scrubbing/washing your hands until I had to scrub for surgery like 5/6 times in a day and my hands aged like 500 years. This one from lush is so rich and creamy and hydrates your skin in a flash! Also there’s something calming about rubbing some nice smelling lotion into your hands

“The paint’s supposed to go where?”

“It’s not paint, dumbass.” Juvia growled as the nurse spread the mystery paint jelly all over Juvia’s slightly swollen stomach.

“Then…what is it?” Gray experimentally poked the gel and then retreated at the foreign texture.

“Ultrasound jam, I don’t know.” Juvia answered. The nurse, Jenny, {one of Juvia’s work friends, as it was} who was barely listening to their conversation, nodded and continued to spread the jello.

“So you are gonna tell me the gender, right? You were joking in the car when you said you wouldn’t?” Gray double-checked.

“If you’re good,” Juvia promised, priding herself in her year of training in the maternity ward.

“So, Gray, how’s it been with cranky-ass ‘Via at home? She’s a nightmare here.” Jenny flirted playfully.

“Fantastic. I got marks on my wrists from her vulture grip, see?” Gray showed off the barely-there red bands across his forearms.

“Stop showing them off! Jenny, do the thing.” Juvia motioned, pointing to the flashlighty-stethescope mabob.

“Alright, fine, just hold on a sec.” Jenny went to input something in the computer while Juvia lamented all of her life choices.

“Oh god…what if something’s wrong with the baby? Maybe I accidentally did pregnancy wrong…” She muttered to herself.

“Aren’t you supposed to be like…an expert on this stuff? Relax, would you? The baby, is perfect.” Gray encouraged with a pat to the un-jellied part of Juvia’s stomach.

“”Yeah Juvia, you’ll get wrinkles,” Jenny teased without looking back. “Sides, you got Swayze over here to purify your disgusting gene pool. There is a fifty-percent chance that your brat is gonna look like him.” Jenny laughed while poking her thumb at Gray. Juvia being married to him was a running joke around the hospital.

“I guess so,” Juvia breathed out, a bit more calm. “I guess we’ll just have to pray that jumping-bean here has more Fullbuster than Lockser.” She whispered to her stomach inconspicuously.

“You’re so rude to yourself! Baby, if you look like Juvia you’ll be fine.” Gray spoke to Juvia’s stomach, hovering his fingers over her coated stomach like he was performing a psychic-reading.

“Baby, don’t listen to Gray! What he lacks in brains he makes up for in everything else.” Juvia whispered the last part to her baby quietly, even though Gray could clearly hear her.

“Stop calling it ‘Baby’! Give it a name, don’t make your baby the star of Dirty Dancing.” Jenny yelled, quite enjoying the show for herself.

“Nobody puts Baby in a corner,” Gray quoted, before laughing.

“No names!” Juvia bit out. “Not yet, at least…”

“Do you want to see the goddamn baby, or not?” Jenny asked, having been waiting for a small amount of time.

Juvia said no at the same time Gray said yes.

Gray looked at her pointedly. “What do you mean, ‘no’, don’t you want to see our precious-“

“-potential disaster? I’m scared, Gray, give me a freakin’ second.” Juvia hyperventilated, rubbing her temples nervously and singing the Star Spangled Banner in her head.

“Ok, ok…” Gray comforted her by placing a hand on her forehead and shutting her eyes, like a peaceful dead body. “Relaaax~”

“I want peppermint ice cream.”

“It’s in the freezer at home, but we have to see our baby before you can have any, ok?”

“Hmm…ok.”

Juvia opened her eyes and nodded at Jenny slowly. Understanding her fear, Jenny eased the scope onto Juvia’s stomach.

“Ok, your baby will be on the screen in three, two…”

“Jenny, I’m not nineteen, you don’t have to count.”

“…one.”

Juvia, breathing heavily, ducked her head to give the hospital floor a good once-over. That was one very nice floor, it was a good floor, a very, very nice floor…

“Juvia, baby, look at it. You have to point out the baby to me because I’ll be damned if those blobs make anything legible…” Gray mumbled, squeezing her shoulder lightly as he watched the ultrasound screen intently. Jenny moved the probe to the center of Juvia’s stomach.

Juvia slowly looked upwards at the screen, blinking carefully in case she saw anything she didn’t want to.

Her eyes adjusted to the screen for a few seconds before she blinked wildly.

“Whoa, Juvia…”

“Jenny…what is…”

“What, what’s going on? I don’t speak nurse, all I see is the beginning scenes from It’s a Wonderful Life, somebody translate!” Gray demanded, voice panicky and excited all at once.

“Jenny what the hell is that?’ Juvia asked, surely her heart rate had spiked as she began to breathe in and out erratically.

“Juvia…I think it’s…”

“No, no!” Juvia screamed frantically. “That can’t be right, I can’t do this, I can’t-“ she looked up at Gray with terror etched into her crystalline blue irises. “Gray punch me in the face! Knock me out, for god’s sake-“

“What the fuck is going on?!?!” Gray yelled dramatically, staring at the shapeless blobs on the screen.

“Punch me in the goddamn face Gray, before I pass out!”

“-calm her down, dammit, she’s gonna have a heart attack-“

“Would somebody just tell me what in Satan’s rectum is going on?”

To conclude the great panic of Juvia’s pregnancy, Juvia promptly passed out, just as she had promised.

“What the fuck was that?!” Gray asked, shaking his wife’s shoulder urgently. “The fuck just happened?”

“What a drama queen,” Jenny said light-heartedly, although her hair and makeup was a bit too frazzled to say that she hadn’t been at least a little bit scared. “Nothing’s wrong, really-“

“Juvia wake up!” Gray yelled, wishing he had a bucket of water, or maybe Gatorade, she hated Gatorade, that’d get her to wakeup… “You need to wake up, I can’t do this without you!”

Juvia’s eyelids moved lethargically and she groaned like she had been in a triple pile-up car accident.

Fine…just…lemme breathe…”

“Oh thank god…now…tell me what’s going on, or I’ll stick that ultrasound probe thingy down your throat and find out for myself…” Gray threatened, mirroring his wife’s unique skill of making up horrific threats on the spot.

“It’d be faster if you went up my-“

“That’s enough, I’ll take it from here, if you don’t mind, Juvia.”  Jenny snarled, readjusting the ultrasound scope to see the blobs once again on screen.

“S-sure, I need to rest for a sec…I might be a little bit sick too…” Juvia muttered.

“Ok Gray, now what you’re gonna see right here is the fetus,” Jenny pointed to a pale whitish-gray blob.

“It’s so tiny,” he breathed, resisting the urge to smash his nose up against the screen to marvel at the itty-bitty wonder.

“And…there’s the other one…” Jenny pointed to another blob, a bit fainter, but still present.

“Other…one…?” Gray gaped, Juvia in turn, paled significantly.

“There’s fucking two of them, Gray. Two.” She choked out.

“Oh my god-

“-I’m going to die-“

“-I’m gonna be a double-dad!”

Jenny stared at them both in complete awe. “How in god’s name are you guys married?”

“We balance each other out.” Gray informed, kissing Juvia’s pale forehead proudly.

“Yes,” Jenny observed, intrigued as Gray gently began to calm his wife down. “…balance…”

“-and we get twice as many presents, twice as many rooms, twice as much fun-“

“-Twice as many bills, twice as much food, oh god, my boobs!” Juvia clutched them in anguish. “They already hurt!”

“I’ll take those,” Gray replied calmly.

“You guys are gonna be fine.” Jenny said crisply, watching Juvia slowly return to her normal breathing rate as Gray told her all the pros of having two babies in her uterus.

Jenny was no genius, but she had been around Juvia long enough to know that it took willpower of steel and patience of titanium to calm that woman down.

-And she pondered the word, balance, perhaps two babies would be enough to balance them out as well.

They’d be alright, just you wait and see. If any two idiots could do it, they could.

Nico had no reason to be in the infermary. Tripping over your own baggy pants and shoe laces was no reason to go have a a very attractive son of Apollo look you over. But here Nico was, and who the hell was his healer? Will Solace. Nico could hardly fight down his blush.

“I still don’t see why I’m in here.” Nico huffed. “I tripped. Big deal.”

Will looked confused. “That’s not why you’re in here. All year rounders need a physical, make sure they’re healthy, stuff like that.”

Nico had to fight  down the blush even more. He hoped that didn’t mean-

“I need you to remove your shirt please.” Will asked, picking up a clip board.

“Do I really need to?” Nico asked, blush starting at his neck and traveling all the way to his roots.

Will looked at him, blue eyes intensely focused. “Well yea. You’d be surprised how many demigods try to hide injuries from me. It’s just a paranoia thing. Hey you okay? You’re concerningly red.”

“I-I’m fine!” Nico said all too quickly. Nico hesitantly started to remove his jacket and folded it. When he looked up Will had a weird look on his face.

“Uh, do I have something on my face?” Nico asked, blush now fading.

Will looked like he was about to freak out. Nico swore he was fine a moment ago.

“Uh, yea. I’m just distracted today.” Will supplied. Nico shrugged. He was over being in this tiny ass room so he pulled his shirt over his head, folded it, and put it with his jacket. Nico hauled himself on the patient examination table and swung his legs idlely.  Will looked half normal again. Will put the stethescope buds in and turned to Nico.

“Okay, deep breathes please.” Will asked, putting the device to the ravenette’s chest. Nico fought the urge to gasp, both at the cold metal and Will’s inhumanly warm hands. 

“Okay lay back please so I can check your ribs and such.” Will asked.

I’d lay back for you anytime. Nico thought to himself, but said nothing.

After several minuets of poking and prodding Will was done, and Nico felt warmer than he’d felt in a long time.

“Everything seems okay, nothing wrong.” Will informed him.

“Awesome.” Nico said as he put his shirt back on.

When Nico’s head poked through his shirt hole, Will looked guilty. Nico wouldn’t stand for that.

“Hey man, are you alright?” Nico asked, stepping closer.

Will was honest, but what he said surprised Nico nonetheless.

“I lied to you.” Will said miserably. “You didn’t need any sort of physical. I just wanted to see you without a shirt. And I feel really bad about it now.”

Nico had no response. He was so flustered, he backed into the shadows and Shadow Traveled away. Will felt even worse.

Two weeks later and Nico still can’t shake the feeling of Will’s too warm hands off his body. 

He’s jacked off who knows how many times, and had far too many cold showers.

Friday the camp was doing war games and Hades Cabin was on the same team as the Apollo Cabin. Nico couldn’t wait. Maybe he would grasp the courage to talk to Will.

Nico’s team was closing in on the opposite teams flag, when some Ares kid decided it would be smart to ambush Nico. Nico won, of course, but not without any injury.

And who happened to get to Nico first?

Will Solace.

This was totally Nico’s moment.

“Hey, you’ll be okay! Let’s get you back to the infirmary.” Will said hastily, moving as quickly and efficiently as he talked.

So here Nico was again, in the same room, shirtless, with Will’s hands moving quickly and perceptively. Nico didn’t really hurt anywhere, but he let Will have his fun.

“Okay, your set. But I really don’t want you going back out there.” Will said, no argument in his tone.

“Alright.” Nico said, slipping on a clean shirt Will offered him. Nico knew this was his moment to tell Will he really liked him, but he opened his mouth and nothing came out. Will looked at him, waiting.

Nico mustered his courage, stepped up to Will and looked him in the eye. Nico didn’t know what to say still. 

“Um, thanks for, uh, taking care of me.” Nico said. Will smiled at him.

“No problem so are we cool now or wh-” Will didn’t finish his sentence, because Nico grabbed Will by his shirt and brought Will’s lips half way to meet Nico’s in a brief kiss.

It lasted only a moment, but it’s all Nico needed to know he was hooked.

When Nico pulled away, Will followed his lips, not done at all. Nico, shocked, gasped, letting Will’s tounge in. Nico moaned, so beyond happy.

“Please don’t be messing with me.” They both said at the same time. They laughed, looked at each other, and continued kissing.
