The illustrator of One Piece doesn't like drawing Death, that's why noone legitimately dies. Except for Whitebeard and Ace, cuz they were vital to Luffy's character development. If someone dies it means they REALLY had to.

Part of it too is that Luffy himself doesn’t like to kill anyone. In one of the SBS volumes, Oda said that Luffy doesn’t kill anyone because the only fate worse than death is being defeated and knowing you can never accomplish your goal/dream. Like killing these villians would be too easy of a punishment. I think that is a large part of why there are not that many deaths in One Piece. Plus it allows for a lot of old characters and villains to come back, even as allies! 

Unless, like you said, they are part of a backstory or necessary somehow for character development.

EDIT: I found the exact (translated) quote from Oda from SBS Volume 4 explaining this:

In this era, people put their lives on their beliefs and convictions, and fight. When he goes into battle, Luffy is destroying others’ beliefs. And when these enemies’ beliefs are shattered and defeated, they feel a pain equal to death. I think that for these pirates, killing or not killing is secondary to winning or losing.

It is also cool to note that this is VERY early on in the series, but the lack of Luffy or even other straw hats killing their enemies is still pretty true. Even though the series has got darker in tone since then.
