025 was brought in with brother 026. 025 was unable to be properly kept at a previous institution and was placed in our care due to a violent outbreak with patient 026 that resulted in an inury for staff at that institution. 025 has not shown remorse for his actions and does not seemed concerned for staff safety.

Doctor’s Notes:

“It appears that 025 has little remorse for his own actions. He does not seem dangerous and is currently calm. He seems to be aware of his surroundings but refuses to speak. 026 is in the room with him because when removed 025 shuts down or becomes agitated. It is in their best interest to be kept together. Other than their appearance and muteness, I see nothing wrong with them. I fully believe that the passing of a previous co-worker they were close to caused their violent outburst. I do not think the brothers need to be admitted within our asylum at this time, or any asylum for that matter.”

Care needs:
Hair wash: 3 times a week.
Full wash: 3 times a week.
Half wash: Any time a full wash is not offered.
Nail care: Once a week.
Cathetar care: N/A
Foot care: Once a month.
Wound care: N/A
Medication: Please see the med charts and med book.
Nutrition: 1800 calories a day. Offer frequent snacks. Please keep an eye on the patient while eating, as patient has a habit of eating too quickly. Ground all meats. Bite-sized peices. If hungry offer sugar-free 0 calorie jello often. Fluids are on no strict intake.
Supply wheelchair: N/A
Bathroom care: If 025 does not have a BM within 3 days offer a MOM-bomb, day 4 give enema, day 5 hospitilization.

Psycho-social:

Counseling: Offer one-on-one counseling twice a month.
Additional notes: Please include 026 in the counseling sessions.
Spiritual support: Offer time in the chapel.
Staff-Patient support: Staff is to initiate conversation with the patient every 30 minutes. If at all possible include physical contact through high-fives or hand-to-hand touching.

Goals:

1. The Patient would like to stay healthy during their time spent within the institution. 025 did not indicate verbally that he would like this, but it is something that should be wanted. This will be achieved with help from staff and by following the POS. This goal will be met monthly. The patient will not have to be hospitalized anytime during the next 12 months.
Additional notes: This goal will continue for the duration of 025’s stay.
2. The patient would like to have less violent outbursts. 025 did not indicate verbally that he would like this, but it is something that should be wanted. This will be achieved with help from staff and by following the POS. This goal will be met yearly. The pateint will not have a violent outburst during the next 12 months.
Additional notes: This goal will continue for the duration of 025’s stay.
3. The patient would like to be more social. 025 did not indicate verbally that he would like this, but it is something that should be wanted as it will increase the value of life. This will be achieved with help from staff and by following the POS. This goal will be met yearly. The patient will communicate with staff daily, will be offered chances to socialize, and will be included in as many things as possible with staff.
Additional notes: This goal will continue for the duration of 025’s stay.

Disciplinary Action:

1. Verbal prompting needs to be offered first!
2. Gentle hand to hand prompting may be used next.
3. Offer to take away free/quiet time.
4. Leave the room and start on the proper forms for the patient’s outburst.
5. Restraint may be used if a cool-down period does not work.
6. A meeting needs to be set up for patient and the doctor to discuss the problem.

Additional Notes: 025 has little to no outburst problems. Please make sure to file a change of behavior form after the outburst. Keep all other patients away from 025 during any outbursts. If at all possible provide 025 with a helmet, as 025 will abuse their face during an outburst.
