This is one of the rare times I have felt sorry for Tobirama. He doubts his brother’s loyalty to him. He isn’t sure if Hashirama will choose to save his life over Madara’s.

It’s easy to imagine why Tobirama distrusts/hates Madara. Tobirama has to compete with him on everything from Hashirama’s time and the conception and development of his dream when they were younger, to later Madara possibly succeeding Hokageship. As the only remaining brother of Hashirama Tobirama stands in line to succeed should something happen to Hashirama. He would either be the next Senju leader or the next Hokage. He was likely used to wielding considerable clout as not only a male of the head family second only to his brother but also as the lieutenant/general of Hashirama.

Tobirama is used to being the main voice his brother listens to.
